The most important
part of a Health Care Power of Attorney is the appointment of someone to
make health care decisions for you if you should be unable to do so. Nothing
else has to be included. However, most persons will want to include additional
statements to accomplish some or all of the following:
· define the scope of your agent's powers or limitations;
·provide guidelines for your agent to follow;
· name successor agents, should your primary agent become unable to act;
· include other directions aimed at ensuring the effectiveness of the document.
